| Q: WHAT IF THERE IS A COUPLE LIVING TOGETHER? |

Our base rates are for a single individual. There is a discount for the second individual in the home, based on the level of care needed.

| Q: WHAT TYPE OF PAYMENT DO YOU ACCEPT? |

We accept assignment from most Long Term Care Insurance companies, as well as personal checks and most major credit cards.

| Q: WHAT ARE THE DIFFERENT TYPES OF LICENSURES IN FLORIDA? |

There are three licensures to provide home care in Florida: Home Health Agency, Nurse Registry and homemaker companion. We are a Nurse Registry, licensed through AHCA (Agency for Healthcare Administration) http://www.fdhc.state.fl.us/index.shtml. Broward County, NR30211081 and Palm Beach County, NR30211094.

| Q: DO YOU PROVIDE SERVICES IN ASSISTED LIVING FACILITIES, INDEPENDENT LIVING FACILITIES, NURSING HOMES AND HOSPITALS |

Yes. People sometimes discover that the assistance provided in these facilities is not enough to enable them to remain comfortable and independent. In these cases, Concepts in Caregiving refers a caregiver to make this possible. In Florida, you are allowed to hire private help in these types of facilities.

| Q: HOW QUICKLY CAN SERVICES BE PROVIDED? |

Generally, services can be provided within two to four hours. Our staff can respond immediately to emergencies.

| Q: CAN I CHANGE THE SCHEDULE IF I HAVE APPOINTMENTS? |

Our philosophy is to make the schedule work for the family not the agency. You come first. Your needs, your requests, your schedule. With adequate notice, usually 24 hours, we will do everything possible to make sure the schedule you want is the schedule you get.

| Q: WILL I NEED TO SIGN A CONTRACT? |

The state of Florida requires that clients sign several forms. Our consent agreements do not obligate you to receive care for any specific period of time. There are several forms that all Long Term Care Insurance companies require.

| Q: DOES ANY GOVERNMENT PROGRAM COVER THE COST? |
A: |
Other than MEDICAID, not MEDICARE, there are no government programs to cover private duty care. To those who qualify there is a VA benefit that possibly would be available. |
